Many people considering facial contouring surgery expect dramatic changes through bone resection alone. However, the degree of change can vary significantly depending on an individual’s unique anatomical conditions. To understand this, it’s essential to look at the various components that define our facial silhouette.

1. The Harmony of Bone and Soft Tissue

The visible outline of our face is determined by the total combination of the facial bones and the soft tissues covering them.

  • Facial Bones: Consist of the skull, cheekbones (mid-face), and jawbone (lower face).
  • Soft Tissues: Anatomically include muscles, fat (subcutaneous and buccal fat), and skin.

As seen in anatomical diagrams, there is a gap between the bone structure and the final skin outline. This gap varies from person to person, which is why bone surgery alone doesn’t always guarantee the same result for everyone.

2. Surgical Methods by Area

  • Mid-face (Cheekbones): Malarplasty (Cheekbone reduction) is used to narrow the face width or soften prominent 45-degree cheekbones.
  • Lower Face (Jaw): Mandibuloplasty (V-line or Square jaw surgery) is for those with a heavy or angular lower face.
  • Chin: Genioplasty addresses concerns like receding chins, long chins, or protruding chins.

3. Improving Soft Tissue Conditions

Sometimes, the soft tissue thickness is the primary factor. In such cases, these methods are used:

  • Liposuction / Contour Injections: Reduces the subcutaneous fat layer.
  • Buccal Fat Removal: Removes deep fat pads under the cheekbones.
  • Botox: Temporarily reduces muscle volume (masseter).
  • Fat Grafting / Fillers: Adds volume to sunken areas for a smoother contour.
